To obtain your official transcript, or to request other records such as test scores, immunization history, or special education records, please contact us with the following information:

First and last name
Date of birth
Address of where the transcript will be sent
Fax number if the transcript is being faxed (Note that it will not be considered official if it is faxed)

Please allow at least two business days to process your request.
